A copy of a resident registration certificate (issued within three months of application, with the address included).
A copy of a foreign registration certificate (within its validity period).
A copy of a disability certificate, rehabilitation certificate, or mental health welfare certificate (within its validity period, showing the page with the name and address).
A copy of a utility bill receipt or invoice (showing the name and address).
